Instructions for Spokane Coaching Logs
Overview
The online coaching activity log involves 3 distinct forms:
· The One-On-One Coaching Log is for use by coaches to document the assistance provided directly to individual teachers.
· The Group Activity Log is used to document activities involving two or more teachers served at the same time.
· The Support Activity Log is used to report time spent that did not involve contact with teachers and which is not recorded in the other log forms.
Note: It is important that any segment of time be reported only once in only one of the 3 forms.

If something does not work as expected, please let us know so that we can fix it. Email emily@facetinnovations.com with as much detail as you can provide about the problem (e.g., what browser are you using, what did you do, what did the error message say).
Procedure for log submission
You may access the online logs at any time to record activities. You might find it helpful to "log as you go"; for example, record activities at the end of a day or after a group event. You may also record activities up to 15 days after the month in which they occurred; for example, if you prefer to write down your notes on paper and transfer them online at the end of the month. Click the 'submit' button to enter data.
You will see a confirmation page that says your log has been successfully submitted if the data has been saved.
If you have incorrectly filled out the form, the errors will appear as tiny
red exclamation points (). If you click on this
exclamation point, a window will pop up describing the error. Data will not be
saved until you edit it and click submit.
Instructions for one-on-one science coaching log
You can use this log to record one or more detailed activities with a teacher, powerful moments with a teacher, or both at once.
Teacher: Each log entry describes activities for one teacher. Select this teacher from the pull-down menu. This selection is mandatory.
Month: You can submit logs up until the 15th of the month after they occur. Therefore, you will have either one or two choices for the month in this pull-down menu. This selection is mandatory.
Activity, Time, Focus and Initiator
For each activity, if applicable, enter a time, focus, and initiator for the
activity. If you click on a little question mark (
), a window will pop up giving
more detail about how to enter the fields. Time should be
specified in hours (e.g. 1h, 1:00) or minutes (e.g. 20m, :20) or in general time
format (1:30). Spaces are not allowed when specifying times (e.g., "1 h" will
give an error). Also, numbers of minutes greater than 99 will not work (e.g.,
"100m" will give an error). Use hours and minutes if specifying times greater
than 99 minutes. Focus is specified by clicking the boxes
corresponding to each focus. You must choose at least one focus, but often
several will apply. Initiator is selected by choosing an option
from the pull-down menu.
Powerful Moments
This is a text box in which you can describe moments or activities that you feel have a strong impact.
Other
This is a text box in which you can describe activities that don't fit in the above categories.
Instructions for group activity log
This log is used to describe activities that involve one or more teachers at the same time. These teachers might be the teachers you coach, or other teachers.
Month: You can submit logs up until the 15th of the month after they occur. Therefore, you will have either one or two choices for the month in this pull-down menu. This selection is mandatory.
Initiator: Select an initiator by clicking on the appropriate radio button. This selection is mandatory.
Approximate number of participants: If specified, this should be an integer that represents the approximate number of participants. Sometimes coaching activities involve a large group of teachers, possibly not your own. This is to give us a sense of whether the activity was large, involving little interaction with teachers, or small (e.g, just you working with three teachers). This selection is not required if it does not seem to make sense for the activity you are describing.
Teacher Participants: This list should accurately reflect
the teachers you are currently coaching. Please inform us at any time if it is
incorrect!
Select the teachers who participated in this group activity, if any, by clicking
on their names to highlight them in the "Available" box on the left and then
clicking on the arrow (>) pointing to the "Selected" box on the right. You
will see their names move over to the "Selected" box. If you make a mistake, you
can move them back. If you want to select all of the teachers on your list, by
clicking on the double arrow (>>) all of the teachers' names will be
selected (and moved to the right hand box).
Activity of coach with group of teachers, Time and Focus
You must describe this activity using at least one of the categories listed here
to submit this form. If it does not fit precisely, select the closest category
and explain in the Other text box below. Time

Note that by default, the Focus for several categories related to student assessment is set to "A" for assessment. This is only to save you some time when filling out the form. You may unselect these or choose different foci for these activities, if appropriate.
Please let us know how these activities relate to larger
initiatives
We are trying to keep track of how group activities relate to larger school or
district-wide initiatives. Two that we are particularly interested in are
data-driven dialogue and lesson study. If the group activitiy relates to one or
both of these, please select these boxes. New initiatives come up from time to
time and we would like to know about them. If the group activity relates to a
new activity, please write in the name of this initiative.

This is a text box in which you can describe moments or activities that you feel have a strong impact.
Other
This is a text box in which you can describe activities that don't fit in the above categories.

enter several at once. If your activity does not quite fit the category, please
